Forms needed by summer band:
- Band Handbook Acknowledgement Form (page 21).
- Student Travel Permission Slip (this covers all band events for the school year).
- UIL Student/Parent 8 hr. Rule Acknowledgement.
- UIL Physical Form. Healthcare provider's signature must be dated after May 15 of the current year.
- EPISD Rules Acknowledgement form.
- Instrument Check-out Form only if needed.
TO DO list for summer band:
- Pay fair shares of $200.
- Donate 2 cases of water (40ct each case).
- Schedule your student's physical.
- Purchase supplies listed in band handbook: Black Drillmaster marching shoes (Olivas Music usually has some in stock), black marching gloves (except percussion), LONG black socks, sheet protectors, lyre, flip folder, athletic shorts or leggings, "blue book" - Foundations for Superior Performance by King and Williams, and if applicable, reeds, valve oil, drumsticks and yarn mallets.
